The simulation allows you to test the financial impact of person-centred care interventions for people living with multiple long term conditions.

Many health systems are developing new models of care to better meet the needs of people living with long term conditions and reduce unwarranted hospital visits. It can be a challenge to predict the likely financial outcome of these new models of care given the complexity of patients and their use of services. Implementing without an understanding of financial and other outcomes is a risk.
